Ageing Foot Care
Professional podiatry care to maintain healthy, comfortable feet as you age.
What is it? The ageing foot is a natural physiological process, much like changes in our skin or hair over time. As we age, the tissues in our feet undergo subtle structural and functional transformations. These changes are cumulative and can significantly impact our comfort, mobility, and independence. In the UK, many older adults face challenges with their feet that, while not life-threatening, can profoundly reduce their quality of life.
The primary underlying reason for many ageing foot problems is Fat Pad Atrophy. The feet are naturally equipped with specialized, dense 'fat pads' under the balls of the feet and the heels. These act as natural shock absorbers, protecting the bones and joints from the impact of walking and standing. With age, this fat tissue naturally thins out (atrophies), leaving the bones and nerves more vulnerable to pressure. Furthermore, the skin of the foot becomes thinner, less elastic, and drier, while the joints may become stiffer, and circulation can often decrease.
What are the symptoms?
- Increased Foot Pain: Feeling the ground more prominently when walking, and general aching, particularly in the balls of the feet and heels, due to loss of cushioning.
- Dry, Fragile Skin: This is very common. Drier skin can lead to painful cracking (fissures), especially around the heels.
- Corns and Calluses: Hard skin often forms over the prominent bony areas created by foot structure changes or fat pad loss.
- Balance Issues: Changes in foot structure and reduced nerve sensation (proprioception) can affect stability, increasing the risk of trips and falls.
- Nail Changes: Nails frequently become thicker, harder to cut, and may change colour (yellowing). This often happens as nails naturally grow slower and are more porous with age.
- Difficulty with Self-Care: Reduced mobility, stiffness, or changes in vision can make it difficult for older adults to safely reach their feet and manage their own nail and skin care.
What causes it? The ageing foot is a direct consequence of the body's natural ageing process:
- Hormonal Changes: Reduced production of natural skin oils (sebum) leads to dryness and fragility.
- Cellular Ageing: Reduced cell turnover rate contributes to skin and nail changes.
- Fat Pad Loss (Atrophy): A natural decrease in dense fat tissue under pressure points.
- Joint Changes: Reduced joint fluid (synovial fluid) and wear on cartilage lead to stiffness.
- Poor Circulation: Arterial issues can lead to reduced blood flow to the extremities, slowing healing and causing other symptoms.
What are the treatment options? It is crucial not to attempt bathroom surgery or ignore persistent pain. A private podiatrist can provide effective solutions, especially when over-the-counter treatments have failed:
- Painless Debridement: This is the most effective and immediate treatment. The podiatrist uses specialised instruments (like scalpel blades) to carefully and painlessly pare down the callus and gently "enucleate" (remove the centre 'plug' of) the corn.
- Custom Orthotics: Bespoke insoles designed from a 3D scan of your feet incorporate metatarsal domes and are designed to correct any underlying biomechanical issues, redistributing your body weight more evenly away from the neuroma.
- Clearanail (Micro-drilling): (Where available) This advanced, non-invasive system uses computer-guided micro-drills to create tiny, painless holes (called micro-pores) through the nail plate, allowing antifungal sprays or lacquers to penetrate deep into the nail bed and directly attack the fungi.
- Laser Therapy: (Where available) A non-invasive treatment that uses laser light to penetrate the nail and destroy the fungi. Multiple sessions are typically needed.
- Manual Therapy and Massage: Hands-on techniques to release muscle tension and improve foot mobility.
- Supportive Footwear: Advice on choosing footwear with good arch support and ample cushioning for all daily activities.
- Extracorporeal Shockwave Therapy (ESWT): A highly effective, non-invasive treatment that uses acoustic waves to stimulate healing in the inflamed tissue, particularly beneficial for chronic, stubborn cases.
